This role offers the opportunity to provide high-quality, patient-focused care to adult and geriatric patients, including underserved populations, within a supportive and collaborative primary care environment.

Job Duties:
- Recuperative care (monitoring vitals, wound care, med pass)
- Care coordination and oversight
- Accurate documentation and communication with care teams
- No assistance with Activities of Daily Living (ADLs)

Qualifications:
- Active California LVN license & BLS certification
- 2+ years of LVN experience required
- Spanish speaking highly preferred
- Experience with adult/geriatric and underserved populations preferred
