Job Title: Licensed Practical Nurse (LPN) – Long-Term Care & Rehabilitation
Job Summary:
The LPN provides direct care to patients in a long-term care and rehab setting, assisting with daily activities, administering medications, and supporting rehabilitation efforts. This role requires monitoring patient health, collaborating with the care team, and ensuring patient comfort and safety.
Key Responsibilities:

Monitor vital signs and administer medications.

Assist with daily living activities (bathing, feeding, dressing).

Provide wound care and basic treatments as directed.

Document patient progress and report changes in condition.

Support patients in rehabilitation exercises and therapy.

Work with the healthcare team to develop care plans.

Ensure patient safety and adhere to infection control policies.
Qualifications:

LPN license and completion of an accredited nursing program.

Previous experience in long-term care or rehab preferred.

Strong communication, clinical, and patient care skills.
