Licensed Practical Nurse (LPN) – Rehabilitation
Job Summary
The Licensed Practical Nurse (LPN) provides direct nursing care to patients in a rehabilitation setting under the supervision of a Registered Nurse (RN) or physician. The LPN supports patient recovery by assisting with medical care, monitoring patient status, and promoting a safe, supportive environment.
Essential Duties & Responsibilities

Provide basic nursing care in accordance with care plans

Monitor and record vital signs, intake/output, and patient condition

Administer medications and treatments as prescribed

Assist patients with activities of daily living (ADLs)

Observe and report changes in patient condition to RN or physician

Assist with wound care, catheter care, and infection control practices

Support rehabilitation goals by encouraging patient mobility and participation

Maintain accurate and timely documentation in patient records

Communicate effectively with patients, families, and interdisciplinary team

Ensure a clean, safe, and organized patient care environment
Qualifications
-
Current LPN license in good standing (state-specific)

Graduation from an accredited practical nursing program
-
CPR/BLS certification (preferred or required)
-
Rehab, long-term care, or skilled nursing experience preferred
