Licensed Practical Nurse (LPN)
Long-Term Care / Rehabilitation
Position Summary
The Licensed Practical Nurse (LPN) provides nursing care to residents in a long-term care and rehabilitation setting under the direction of a Registered Nurse (RN) or physician. The LPN is responsible for delivering safe, effective, and compassionate care while supporting residents’ medical and rehabilitation needs.
Essential Duties & Responsibilities

Provide direct nursing care to residents in accordance with care plans and physician orders

Administer medications and treatments as prescribed

Monitor, assess, and document resident conditions and report changes to RN or physician

Perform wound care, dressing changes, and basic treatments

Assist with admissions, discharges, and transfers
-
Supervise and delegate tasks to CNAs and support staff
-
Ensure accurate and timely documentation in medical records
-
Follow infection control, safety, and HIPAA regulations
-
Communicate effectively with residents, families, and interdisciplinary team members
-
Respond to emergencies and initiate appropriate interventions per facility protocol
Qualifications
-
Current LPN license in good standing (state-approved)

Graduate of an accredited practical nursing program
-
Long-term care or rehabilitation experience preferred

Strong clinical, organizational, and communication skills
-
Ability to work flexible shifts including weekends and holidays
