Location: Mohawk Valley Rehabilitation and Nursing Center (RNC), 1650 Champlin Avenue, Utica, NY 13502
Employment Type: Travel and Local Candidates Accepted
Schedule: 8-hour day shift, 6:30 AM – 2:30 PM
Contract Length: 13 weeks
Guaranteed Hours: 37.5 hours per week
Weekend Requirement: Every other weekend required
Floating Requirement: Must float to all units within the same building
On-Call: No

Job Summary
Mohawk Valley Rehabilitation and Nursing Center is seeking a compassionate and skilled LPN / LVN to join our Long Term Care team in Utica, New York. This opportunity is ideal for a licensed nurse who is experienced in providing high-quality patient care in a skilled nursing or long-term care setting. The selected candidate will support residents with diverse needs, including multiple sclerosis, orthopedic care, dementia, and memory care.
This position offers a consistent day shift schedule, competitive assignment length, and the opportunity to work in a supportive care environment serving a 40-bed unit.
Key Responsibilities
- Deliver direct nursing care to residents in a long-term care setting
- Administer medications and treatments according to facility standards
- Monitor patient conditions and report changes promptly
- Collaborate with interdisciplinary care teams to support resident outcomes
- Document care accurately in the Point Click Care EMR system
- Float to all units within the same building as needed
- Maintain a safe, respectful, and compassionate care environment
Required Qualifications
- Active New York LPN license
- Current BLS certification
- Minimum 1 year of nursing experience
- Experience in Long Term Care, Skilled Nursing Facility (SNF), or similar setting preferred
- Ability to workday shift hours
- Availability for every other weekend
- Strong communication, documentation, and patient care skills
