LPN / LVN – Long-Term Care | 13-Week Contract
Facility: Mohawk Valley – Rehabilitation and Nursing Center (RNC)
Location: 1650 Champlin Avenue, Utica, NY 13502
Department: MVHS-RNC-Barton East
Specialty: Nursing – Long-Term Care
Contract: 13 weeks | September 14, 2026 – December 11, 2026
Shift: 2:30 PM – 10:30 PM | 8-hour evening shift
Guaranteed Hours: 37.5 hours/week
Scheduling: Sunday–Saturday | Every other weekend required
Local & Travel: Both accepted; local candidates within 75 miles eligible for local rate
Position Overview
Mohawk Valley – Rehabilitation and Nursing Center is seeking an experienced LPN/LVN for a 13-week Long-Term Care assignment. The position provides nursing care to residents with a range of conditions, including multiple sclerosis, orthopedic needs, dementia, and memory-care needs.
Requirements
- Minimum 1 year of Long-Term Care (LTC) experience
- Travel experience must be documented on the resume
- Current BLS certification
- Active New York LPN license
- All active licenses must be verified through a current Nursys report
- Current driver’s license with an address matching candidate demographics
- Skills checklist matching the specialty
- Experience with PointClickCare
- RTO must be submitted at the time of submission
Unit Details
- Unit: LTC
- Beds: 40
- Common Diagnoses: Multiple sclerosis, orthopedic conditions, dementia, and memory care
- Floating: Required within the same building
- Scrub Color: Navy blue
- Block scheduling: Not promised or guaranteed
Overtime
- Daily overtime after 24 hours, based on multiplier
- Weekly overtime after 48 hours, based on multiplier
LTC experience is required. Local candidates are accepted at the applicable local rate.
