Salary: $200,000 - $250,000

Job Summary

We are seeking a friendly and compassionate Travel Nurse Practitioner to join our team, supporting the onboarding of patients into the Patient Health Management (PHM) program. In this role, you will work closely with care coordinators across various providers and settings to promote improved health outcomes. You will facilitate patient assessments, assist with communication between facilities and patients, and ensure accurate documentation through our proprietary software system. This is an exciting opportunity to make a meaningful impact on patient health outcomes while working in a dynamic and supportive team environment.

Responsibilities

  • Onboard patients into the PHM program via telehealth – benefits- informed consent.
  • Assist care coordinators across the different providers and settings.
  • Promote better health outcomes through communication between facilities and patients.
  • Assess patient condition and reporting through our proprietary software system.
  • You will be cross-trained in other areas of care management.
  • Gathering and analyzing patient information
  • Detailed review of the system
  • Review of all pertinent patient labs, studies, and provider notes.

Requirements

  • Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license
  • State-specific license in good standing in Massachusetts
  • Minimum 6 months of experience in long-term care or a skilled nursing facility
  • Strong communication, clinical, and critical thinking skills
  • Passion for elder care and person-centered healthcare
  • Reliable transportation
  • Must be willing to travel to different nursing homes up to 100% of the time
