Job Description

Seeking a Full-Time Permanent Nurse Practitioner for a Breast Care Program!

Position Details:

The Advanced Practice Navigator will incorporate the role of advanced practice providers into clinical practice, education, research, and leadership in addressing the health care needs of all the patients in the Breast Care Program. This position will serve as the conduit for services to provide a patient with the experience of integrated care. The Advanced Practice Navigator facilitates the patients; interactions throughout the care continuum and among service lines including the physician, physician offices, outpatient providers, radiologists, tertiary care centers, other specialties. They coordinate care by guiding the patient through the care process and connecting the patient with appropriate services.

Minimum Education:

- Master's Degree in Nursing and graduate of accredited Nurse Practitioner program

Minimum Work Experience

- 2 Years of NP experience

Licenses/Certifications:

- Board Certified Nurse Practitioner by ANCC
