PAY: $130,000 – $170,000 per year + Relocation Assistance + Referral Bonus up to $1k

About the Role
NurseAmerica is seeking a motivated Primary Care Nurse Practitioner (NP) to join an innovative program delivering primary care directly within dialysis centers. As a founding NP, you will help shape this groundbreaking initiative for patients with end-stage renal disease (ESKD), providing comprehensive, patient-centered care in a convenient clinical setting.

  • Work across a portfolio of 3–8 dialysis centers in the Norfolk/Chesapeake area.

  • Clinical autonomy to manage patient visits, averaging 10–12 patients per day.

  • Schedule primarily 8:00 AM – 5:00 PM to align with dialysis appointments.

Key Responsibilities

  • Conduct annual comprehensive health evaluations and patient follow-ups.

  • Coordinate care with nephrologists, dialysis teams, primary care providers, and other specialists.

  • Order and interpret labs, prescribe medications, and manage chronic conditions.

  • Support development of the program and provide feedback to leadership on care delivery.

Qualifications
Required:

  • Minimum 5 years of clinical experience in Primary Care, Internal Medicine, or Gerontology.

  • VA Licensed Nurse Practitioner with autonomous practice authority.

  • DEA license and experience prescribing for chronic diseases, including congestive heart failure.

Preferred:

  • Experience in dialysis or nephrology.

  • Expertise in managing chronic conditions like diabetes, hypertension, COPD, and heart disease.

Proven ability to initiate, titrate, and monitor medications in accordance with clinical guidelines.
