Key Responsibilities:

Round on patients to assess conditions and manage chronic kidney disease (CKD)
Develop and implement individualized care plans in collaboration with physicians and care teams
Prescribe and manage medications according to clinical protocols
Educate patients and families on treatment options, disease prevention, and wellness
Conduct patient evaluations and provide necessary clinical interventions
Maintain accurate and up-to-date electronic health records (EHR)
Participate in quality improvement initiatives and ongoing professional development

Qualifications:

Master’s degree in Nursing or equivalent
Active Nurse Practitioner license in the state of practice
Experience or training in CKD management and dialysis care
Strong clinical judgment, communication, and organizational skills
Ability to work effectively within a multidisciplinary healthcare team
Proficiency with electronic medical records (preferred)
