Overview:
Our specialized dialysis practice is seeking a Nurse Practitioner to provide comprehensive care for patients with chronic kidney disease. This role involves care planning, clinical management, and collaboration with a multidisciplinary team in a patient-centered outpatient setting. New graduates and experienced providers are encouraged to apply.

Position Offers:
• Monday–Friday schedule with flexible 4- or 5-day options and Friday half-days
• Competitive compensation: $135,200–$176,800 annually DOE

Key Responsibilities:
• Manage chronic kidney disease patient care and ongoing treatment oversight
• Develop and adjust individualized care and treatment plans
• Conduct patient evaluations and provide clinical interventions
• Prescribe and manage medications within clinical guidelines
• Educate patients and families on disease management and treatment options
• Maintain accurate and timely electronic medical records
• Collaborate with physicians and interdisciplinary care teams
• Participate in quality improvement and care optimization initiatives

Qualifications:
• Active Nurse Practitioner license in California
• Master’s degree in Nursing
• Strong clinical judgment and patient management skills
• Effective communication and teamwork abilities
• Ability to work in a multidisciplinary clinical environment
