Key Responsibilities:
Manage patients with diabetes, hypertension, and hyperlipidemia
Conduct consults and follow-up visits
Manage and adjust medications, including insulin titration
Provide patient education and support for chronic disease management
Collaborate with providers and staff to optimize care plans
Maintain accurate patient records and documentation
Ensure compliance with clinical guidelines and protocols
Qualifications:
3+ years of clinical experience as a Nurse Practitioner
Proven experience in diabetes management
Strong clinical assessment and patient management skills
Excellent communication and collaboration abilities
