Hours: Mon- Fri, 8AM- 4:30PM
Employee and dependents 8 years and up
Setting: Primary care, urgent care, vaccines, and lab services, Episodic, Immunizations, Flu Vaccination
LOCATION: Whiting, IN
When: Aug 29 to Nov 30
Shifts: 5am to 1:30 pm with an unpaid 30-minute lunch
Duties: General Primary Care, Acute Care, DOT Drug Testing, Light Occ Med
Ages 2 years and up (Employees and their dependents)
REQUIREMENTS
Active IN license
Active DEA
BLS certification
EMR - EPIC
