One of Southern California’s leading healthcare systems is seeking family medicine nurse practitioners to work at West Covina or Commerce, CA clinic locations as a transition of care and primary care hybrid provider. The successful candidate will work alongside an interdisciplinary team. Candidates must be certified.

The typical schedule is 4 ten-hour shifts with the varying transition of care and primary care duties splitting the nurse practitioner’s day.
