Job Description
Nurse Practitioner needed for Home Visits in Modesto and Stockton! This is a full time, permanent position offering excellent benefits and a competitive wage to include bonus plans. In this role you will be conducting 45 minute to one hour visits for a national in-home visit organization. In this specific role you will be mostly dealing with chronically ill patients, managing those patients and carrying a panel. All mileage is reimbursed at the IRS rate.
