Leadership position!

We are looking for a Physician Assistant or Nurse Practitioner to join our group full time as our Clinical Program Coordinator in South Bend, IN. The Clinical Program Coordinator is responsible for assisting Clinical- Operations with the ongoing assessment and improvement of the Clinical Services Team. Such responsibilities encompass not only the internal functioning of the department, but also the integration of the department into the overall functioning of the organization and its clinical programs.

The Clinical Program Coordinator (CPC) is the primary resource leader for designation location(s) and shall oversee and work in collaboration with the customer and clinical team members to ensure; optimal functioning and performance of the clinical program(s) and their components and day to day clinical operations as directed by their supervisor. The CPC will also assume responsibility and accountability for ensuring patient evaluation and management is completed in accordance with established policies and procedures and maintaining the highest degree of quality patient care. The CPC assumes and is responsible and accountable for the care of patients requiring assessment and treatment within the program. When applicable may be assigned to orient staff and students. The CPC accurately streamlines patients’ needs; assists in identifying and scheduling appropriate clinical interventions; provides patient and family education; and works cooperatively with other clinical team personnel in maintaining standards for professional clinical practice and interdisciplinary teamwork. These functions are carried out according with company policy and in accordance with the mission and vision of the company and consistent with the philosophy of the medical operations. The CPC participates in performance improvement and program development activities and is responsible for overseeing selected activities. Responsibilities include providing input and assistance in oversight into the clinical program components, budgetary planning process; contributing to cost effectiveness of services and programs provided by the medical operations department; communicating the employee’s observations and suggestions related to opportunities for improvement; and maintaining positive relations with all customers.

Essential Duties and Responsibilities:

  • Optimize patient care by coordinating and overseeing clinical staff in the following activities: initiating referrals; evaluating and treating referred patients; complying with billing procedures/systems; establishing and implementing protocols and order sets;
  • Ensure the clinical team effectively participates in the care planning process; appropriately documenting having information to communicate to the patient and family about their status, needs, and to other professionals as appropriate.
  • Submits billing, payroll and expense documentation accurately and timely (within the same day). Assists in the collection of statistics as directed.
  • Provides treatment in accordance with established standards of practice, department procedures and efficiency standards.
  • Establishes individual patient and team schedules in conjunction with other team members. Oversight of medical team’s treatment plans and problem list to ensure appropriate follow up.
  • Coordinating treatment plan with patient, family and other team members and monitoring patient’s response to intervention.
  • Facilitate and assist in appropriate documentation of visits to ensure timely treatment, coordination of care and regulatory compliance. Facilitate discharge planning with medical/clinical professional staff.
  • Supervises other members of the program at same or similar license level, students and volunteers.
  • Reviews the technical quality components of total services delivered and oversee clinical programs for effectiveness and efficiency, using predetermined criteria.
  • Participates in patient, family and staff education.
  • Confers formally and informally with other team members in coordinating the total medical care of the patient.
  • Adheres to our policies and procedures regarding documentation of services.
  • Attend and or coordinate in-service training and facility meetings as required and provides that information to other team members as necessary.
  • Identify need for clinical programs through networking within our members and customers. Develops and assists in providing in-service education and clinical information to the members of the patient care team and to external customers when needed.
  • Assure quality analysis of outcome data and provide leadership in effectively utilizing this data to enhance clinical services and program
  • Support implementation of core clinical programs, clinical protocols, and standards of practice and care delivery systems.
  • Participate in quality review activities to assure appropriate clinical delivery of services and standards of practice are met.
  • Assumes responsibility for ongoing continuing education and professional development. Promote our company as a national presence in professional conference/seminars and publications. Represent the company in professional meetings and national organizations.
  • Maintains a safe and clean work area and adheres to facility/corporate safety standards.
  • Complies with all Infection Control, Universal Precautions, and OSHA standards for the healthcare professional.
  • Consistently demonstrates sound judgment in the evaluation, planning, implementation and follow-up of the patient clinical programs.
  • Maintains positive relationships and rapport with customers, coworkers, patients, family members and facility personnel.
  • Reports to work on time and coordinates schedule to achieve maximum efficiency during assigned shift. Ability to work varying hours, including weekends and holidays.
  • Adhere to our companies Code of Conduct and Business ethics policy including documenting and reporting responsibilities and provides support to regulatory and compliance activities
  • Assists with and participates in Continuous Quality Improvement Program as required.
  • Adheres to established confidentiality standards as required by our company concerning community and patient information.

Requirements:

  • Proven experience as a Nurse Practitioner/Physician Assistant in a clinic, hospital, ER or other setting
  • Ability to maintain a leadership role
  • Expertise in primary care, acute care and chronic care practices
  • Valid nurse practitioner or physician assistant license in IN
  • Master’s degree (graduation from an accredited NP or PA program)

Schedule:

  • Full time

Total Compensation:

  • $110-$120k per year
