Nurse Practitioner - Wellness / Senior Care - Temecula, California

-Seeking NP or PA to join a well-established group for Wellness checks (this is not a primary care position)
-Provides annual health assessments for patients in a variety of settings such as in our Wellness Clinics and at the patients home
-Assesses members for preventative interventions such as safety, fall risk, depression screen, dementia, and urinary concerns.
-Goal is to see up to 12 patients/day, but average is probably 8.
-Home Visit Goal: 6-8 members per day
-Wellness Clinic Goal: 12 members per day
-Permanent; full-time
-Monday " Friday
-8am " 5pm

-Base salary $130,000/year " based on experience
