Employee Health Clinic - seeing employees and their dependents

8/24 thru 10/10/26

Monday: Open 6-3 Lunch 11-12

Tuesday: Open 10-7 Lunch 1-2

Wednesday Open 6-3 Lunch 11-12

Thursday: Open 10-7 Lunch 1-2

Friday: Open 8-5 Lunch 12:30-1:3

Clinic sees 2 years & up.

Requirements of the Nurse Practitioner Family/Primary Care (PRN) Locum role:

• KY state license; active, unrestricted. DEA.
• Board Certified.
• Shadow Training: 2-4 hours prior to first shift.
• CV must be formatted with MM/YYYY for all work w/all work and training.
