Job Description
Company:
- Found in 2002, the FQHC is a team setting that provides compassionate care regardless of income status
- This health center has 5 locations, 25 providers, and 400 members with solid tenure with many, 15 yrs to 20 years

- Salary, depending on experience + incentive bonus program of up to $187k

Your Role with the Company:
- Full time - 3/12s is an option
- APP will work at one location - open location: Fitchburg
- FNP / PA will work 2 weekend days a month
- APP will work in an urgent care seeing all ages
Background Profile:
- Board Certified FNP or PA
- 2+ years of experience as an Advanced Practice Provider
- Active MA License
