Job Description

Position Summary

The Nurse Practitioner/Physican Assistant will provide quality health care and emergency treatment to patients. Health care includes: health promotions; prevention of disease and disability; identification of health problems; management of health problems; and referral with communication to the Patient’s Primary Care Provider. The Nurse Practitioner will practice medicine within the scope of their licensure and if applicable, under the Supervision of a Physician.

RESPONSIBILITIES

1. Provides patient care within established protocols, policies and regulations of the scope of practice to assure quality patient care.

2. Maintains accurate and complete patient health records to assure confidentiality and compliance with health care regulations.

3. Interprets health data by identifying risk factors and provides wellness activities.

4. Diagnosing and treating common diseases and human responses to actual and potential health problems.

5. Counseling individuals and families regarding health care issues.

6. Consulting and/or collaborating with other health care providers and community resources.

7. Referring client to other health care providers and resources for identified health issues.

8. Performs additional duties or projects as assigned by the Chief Medical Officer.

9. Supports the pre-visit planning process and prepares for the days schedule with a daily team huddle with provider.

10. Participate in the Clinical Care Team as outlined in Team Approach to Care Protocol.

11. Work as part of the Clinical Care Team to ensure effective and quality care is given to the patient.

12. Adheres to all policies and protocols.

13. Complies with all safety rules and protocols. Reports all incidents, regardless of severity, immediately to their supervisor and completes an incident report and investigation form, which will be forwarded to Human Resources within 24 hours of the incident.

14. Conducts themselves as a good steward in the communities served by represents the organization at outreach activities as needed.

15. Actively participates as committee member as assigned by the Chief Executive Officer.

16. Interacts harmoniously and effectively with others. Focusing on the attainment of organization goals and objectives through a commitment to team work.

17. Conforms to acceptable attendance and punctuality standards as expressed in the employee handbook.

18. Abides by the organization’s compliance program and requirements

EDUCATION AND EXPERIENCE

1. Graduation from a NLN accredited Nurse Practitioner program, MSN preferred. Certification by American Academy Nurse Practitioners. Active State of Maine Certified Nurse Practitioner License, current CPR, ACLS, or a combination of education and experience.

2. Licensed with the State of Maine.

3. Possess strong communication, analytical, and observation skills.

4. Demonstrate attention to detail and timeliness with documentation.

5. Maintains all current credentialing requirements.

6. Must possess a clean criminal and DHHS background.

7. Must possess the ability and means to travel between clinics as needed.

8. Valid State of Maine Driver’s license and ability to be insured by vehicle insurance carrier.
