A Nurse Practitioner is needed in Homosassa Springs, Florida.

Homosassa Springs offers a perfect balance of outdoor recreation, a growing community, and access to Citrus County. This opportunity is with a mission-driven healthcare organization providing proactive, preventive primary care services to geriatric patients in skilled nursing facilities (SNFs), assisted living facilities (ALFs), and private residences.

Salary and Benefits

• $100,000 base salary annually for meeting standard patient expectations
• Lucrative bonus structure:
  • Approximately $7,000 extra for adding just one additional patient per day
  • High earners make $120,000 to $160,000 annually; top performers have earned about $200,000
• Health insurance (cost shared by employer and employee)
• Dental, vision, life, and disability insurance (available at employee’s cost)
• Malpractice insurance provided
• PTO – 12 days per year
• Paid holidays – 6 per year
• Retirement – SIMPLE IRA with employer contribution
• CME allowance – $1,000 annually

Responsibilities

• Provide on-site primary care services to patients in private residences
• Conduct comprehensive assessments and manage chronic conditions to reduce hospitalizations
• Collaborate with facility teams, patients, families, and support staff to optimize care
• Conduct telehealth visits when needed
• Participate in Medicare’s Merit-based Incentive Payment System (MIPS)
• Attend weekly provider meetings virtually via Teams
• Maintain accurate patient records in eClinical Works (EHR)
• Participate in an after-hours on-call rotation
• Production Goals:
  • 9 visits per day for home visits

Requirements

• Active Nurse Practitioner (APRN) license in Florida
• DEA license
• At least 1 year of geriatric or primary care experience preferred (not required)
• Experience with eClinical Works (EHR) preferred
• Strong clinical skills, communication skills, and ability to work independently
