Nurse Practitioner

Introduction:
We are seeking a dedicated and skilled Nurse Practitioner to join our growing team. This role offers the opportunity to deliver high-quality, patient-centered care in a collaborative and supportive environment that values professional development and clinical excellence. This position services North Westchester County, NY, with travel to one assigned location per day.

Compensation:
- $140,000 – $170,000 annually, based on experience

Schedule:
- Monday through Friday
- No weekends required

Job Responsibilities:
- Perform routine physical examinations and wellness visits for residents
- Manage acute and chronic conditions, including diabetes, infections, and wound care
- Order and interpret diagnostic tests such as laboratory work and X-rays
- Prescribe medications in accordance with state regulations
- Collaborate with interdisciplinary care teams to develop and update individualized care plans
- Educate residents and family members on health conditions and treatment plans
- Accurately document care and clinical findings in the Electronic Medical Record (EMR)

Qualifications:
- 3+ years of Nurse Practitioner experience
- Experience in geriatrics or Skilled Nursing Facilities preferred
- Willingness and ability to travel to one assigned location per day
- Strong communication, interpersonal, and patient education skills
