Responsibilities of the Family Practice APP: Collaborate with the medical director and supervising physician to deliver primary care commensurate with training to patients 18 and upDeliver care appropriate for an out-patient, primary care community health center with a multicultural, multilingual patient populationWork collaboratively with the interdisciplinary treatment team to provide exceptional patient care.Assist in chronic disease management through individual interventions, clinic support and ongoing education.Work collaboratively with the clinic staff to provide high quality, comprehensive primary care.Ability to provide a variety of services including chronic disease management, physicals, perform Pap smear/women's wellness exams (WWE), routine check-ups, illness treatment, medication management, etc.
