Responsibilities:
- Conduct patient evaluations, including medical history reviews and clinical assessments
- Develop, implement, and manage personalized treatment plans
- Prescribe medications and therapies in accordance with legal and clinical guidelines
- Monitor patient progress and modify care plans when necessary
- Educate patients on prevention, wellness, and treatment adherence
- Maintain accurate and timely medical documentation
- Participate in quality improvement initiatives and evidence-based best practices
- Collaborate with interdisciplinary healthcare team members to ensure high-quality patient care

Qualifications:
- Active Arizona Nurse Practitioner license
- Graduate degree from an accredited Nurse Practitioner program
- Minimum of 1 year of experience working as a Nurse Practitioner
- Strong clinical judgment, communication, and organizational skills
- Correctional healthcare experience preferred
