Key Responsibilities:

Conduct patient evaluations, including medical history reviews and clinical assessments
Develop, implement, and manage individualized treatment plans
Prescribe medications and therapies in accordance with legal and clinical guidelines
Monitor patient progress and adjust care plans as needed
Educate patients on prevention, wellness, and treatment adherence
Maintain accurate and timely medical documentation
Participate in quality improvement initiatives and evidence-based best practices
Collaborate with interdisciplinary healthcare team members to ensure high-quality patient care

Qualifications:

Active Arizona Nurse Practitioner license
Graduate degree from an accredited Nurse Practitioner program
Minimum of 1 year of experience as a Nurse Practitioner
Strong clinical judgment, communication, and organizational skills
Correctional healthcare experience preferred
