We are seeking an experienced Nurse Practitioner to provide high-quality primary care services in a fast-paced, patient-centered environment in Brooklyn, NY. Key Responsibilities:

Conduct comprehensive physical exams and patient assessments
Diagnose and manage acute and chronic medical conditions
Prescribe medications and evidence-based treatments
Develop and implement individualized care plans
Order and interpret diagnostic tests and lab results
Educate patients and families on prevention and wellness
Collaborate with physicians and care teams to ensure coordinated care
Maintain accurate and timely patient documentation

Qualifications:

Active and unrestricted Nurse Practitioner license in New York
2–3 years of experience in a primary care setting
Medicaid credentialing required
Fluent in Spanish (required)
Strong clinical, communication, and documentation skills
