We are seeking a Nurse Practitioner to provide comprehensive primary care in a collaborative, multidisciplinary setting. This role focuses on patient assessments, chronic disease management, and preventive care while offering a flexible full-time schedule to support work-life balance.

Key Responsibilities:

Conduct patient assessments and develop individualized care plans
Manage medications and adjust treatments as needed
Provide wellness exams, sick visits, and chronic disease management
Educate patients on preventive care and treatment plans
Order and interpret diagnostic tests
Maintain accurate and timely patient documentation
Collaborate with physicians and care team members

Qualifications:

Master’s degree in Nursing or related field required
Active Arizona Nurse Practitioner license (FNP or AGNP)
Experience or training in primary care or chronic disease management
Strong clinical and communication skills
Ability to work in a collaborative care environment
