Key Responsibilities:

Perform comprehensive patient assessments, including exams and medical histories
Diagnose and manage acute, chronic, and episodic conditions
Develop and implement individualized care plans and manage medications
Order and interpret diagnostic tests to guide treatment decisions
Educate patients and families on disease prevention and health management
Maintain accurate and timely medical documentation
Collaborate with physicians and care teams to optimize patient outcomes

Qualifications:

Active Nurse Practitioner license in Michigan
Certification as FNP or AGNP
Experience in primary care preferred
Strong clinical judgment and medication management skills
Excellent communication, organizational, and interpersonal abilities
Ability to work independently and within a team setting
