Location: Middletown, NY & Port Jervis, NY

Overview:
Our Nurse Practitioner role supports two clinical sites in Middletown, NY and Port Jervis, NY. Our practice delivers comprehensive primary care services focused on geriatric and chronic care populations with strong interdisciplinary collaboration. Position Offers:

• Monday–Friday, 9am–5pm
• Compensation: $140,000–$160,000 annually (DOE)

Key Responsibilities:

• Conduct routine physical exams and preventive care visits for residents
• Diagnose and manage acute and chronic conditions, including diabetes, infections, and wound care
• Order and interpret diagnostic testing and prescribe medications as appropriate
• Develop and update individualized care plans in coordination with the care team
• Educate patients and families on treatment plans and ongoing health management
• Document patient encounters accurately within the electronic medical record
• Coordinate care across two clinical locations with required daily travel between sites

Qualifications:

• 3+ years of Nurse Practitioner experience
• Geriatrics or skilled nursing facility experience preferred
• Willingness to travel between Middletown and Port Jervis locations
