Location: Fremont, Warren, Cleveland, Stow, Marion, Newark, OH

Overview:
Our well-respected Geriatric practice is seeking multiple Nurse Practitioners to join our team on a full-time or part-time basis across several locations. This opportunity offers flexible scheduling and a supportive clinical environment focused on providing care to geriatric patients in the community.

Position Offers:
• Schedule: Monday–Friday 8:00 AM–5:00 PM
• Full-time compensation: $115,000–$130,000 annually (DOE)
• Part-time compensation: $78,000–$96,000 annually (DOE)

Key Responsibilities:
• Provide geriatric patient care across multiple practice locations
• Perform comprehensive patient evaluations and ongoing care management
• Develop and manage individualized care plans
• Monitor patient conditions and adjust care as needed
• Collaborate with physicians and care team members
• Educate patients and families on care plans and health management
• Maintain accurate and timely clinical documentation

Qualifications:
• Nurse Practitioner certification
• Ability to work in a geriatric care setting
• Strong communication and patient education skills
• Ability to work independently and collaboratively
• Flexible availability for full-time or part-time scheduling
