Overview:
Our well-established Family Care Practice in Brooklyn is seeking a Nurse Practitioner to join a collaborative primary care team serving a diverse adult and family patient population. This role focuses on providing comprehensive, continuous care in a busy outpatient setting while supporting improved access and quality of care.

Position Offers:

• Monday–Friday schedule
• No weekends required

Key Responsibilities:

• Perform patient assessments, exams, and risk screenings
• Diagnose and manage acute and chronic conditions in primary care
• Develop and adjust treatment plans, including prescribing and ordering tests
• Provide preventive care, education, and lifestyle counseling
• Coordinate referrals and follow-up with specialists and community resources
• Review and communicate lab and diagnostic results
• Maintain accurate EHR documentation
• Participate in team meetings and quality improvement activities

Qualifications:

• Active or eligible Nurse Practitioner license in New York State
• Master’s or Doctorate in Nursing from an accredited NP program
• National NP certification (AANP or ANCC)
• Current NYS RN license
• BLS required; ACLS preferred
