Location: Merritt Island, FL

Overview:
Our well-established outpatient primary care practice in Merritt Island, Florida is seeking a Nurse Practitioner to join its growing team. This role provides primary care services including Medicare wellness visits, annual physicals, and transitional care in a collaborative outpatient setting focused on quality outcomes and continuity of care.

Position Offers:
• Monday–Friday schedule (8:00 AM–5:00 PM)
• Competitive compensation: $47–$50 per hour (DOE)

Key Responsibilities:
• Provide well visits, annual physicals, and Medicare wellness visits
• Manage post-hospital transitional care and sick visits
• Perform pre-operative clearances
• Provide women’s health services if desired
• Address Medicare HEDIS measures and care gaps
• Deliver care in a collaborative outpatient setting
• Maintain accurate clinical documentation

Qualifications:
• Master’s or Doctorate in Nursing from an accredited NP program
• Current NP certification (AANP, ANCC, or equivalent)
• Active DEA registration or eligibility to obtain
• Minimum 1 year NP primary care experience
• Knowledge of Medicare HEDIS measures and care gaps
• Medicare and Blue Cross credentialing preferred but not required
