Sumo Medical Staffing is recruiting an experienced Nurse Practitioner-Hospice Locums needed in Grand Junction, Colorado (CO). The team is anticipating coverage 8/31/2026 for a 13-week assignment, with ongoing potential. The role offers a competitive market rate and Locums benefit.

51215– Hospice (NP) - PACE Locum Role:

· Coverage: 4x10 hour shifts on site. No holiday coverage.

· PACE (Program of All-Inclusive Care for the Elderly)

· Setting: Primary Care Clinic with in-home visits as needed

· Scope:

· Provide primary, preventive, and urgent care.

· Assess patients across multiple care settings.

· Collaborate with the PACE care team.

· Coordinate community referrals.

Requirements of the Hospice (NP)-PACE Role:

· CO state license: active, unrestricted (in-hand).

· Board Certification

· Experience: Minimum 2 years

· DEA, ACLS

· CV (MM/YYYY, 30+day gaps explained – work and education)
