We’re excited to share a NEW permanent Family Nurse Practitioner opportunity near Medford, Oregon, providing high-quality care to older and high-risk patients throughout the community.

- 💰 Pay: $125,000–$150,000/year

- 🏥 Outpatient + In-Home Care
- 👵 Senior & High-Risk Patient Population
- 🕐 Full-Time | Monday–Friday | 8 AM–5 PM
- 👩⚕️ 8–10 clinic patients/day | 5–6 home visits/day
- 🚗 Local travel required — up to 35 miles

🔹 Daily Responsibilities
As an FNP, you’ll evaluate and manage senior and high-risk patients, conduct wellness exams, review medications, perform screenings, develop individualized care plans, and coordinate follow-up care. You’ll also provide patient education, support transitions after hospital discharge, collaborate with an interdisciplinary care team, and provide care in both clinic and home settings.
🔹 Requirements
- Current Oregon NP License & Certification
- 2+ years NP experience in Primary Care, Family Medicine, Internal Medicine, Gerontology or Palliative Care
- DEA/Prescriptive Authority
- CPR/BLS certification
- Oregon Driver’s License
