🔹 Daily Responsibilities
As an FNP, you’ll manage a panel of senior patients and provide comprehensive primary care, including wellness visits, acute and chronic disease management, physical exams, screenings, medication management, diagnostic testing, and preventive care. You’ll also develop treatment plans, make specialist referrals, conduct post-hospital follow-ups, provide patient education, coordinate care with the clinical team, and maintain accurate EHR documentation and HCC coding.
🔹 Requirements
- Active Montana FNP or AGNP License
- 3–5+ years of independent Primary Care experience
- CPR/BLS Certification
- Montana Prescriptive Authority & DEA License
- Valid Montana Driver’s License
- Adult Gerontology certification preferred
- Value-based care, panel management, and EHR experience preferred
