Job Description
Client seeks Nurse Practitioner for contract assignment starting October 12-December 28 at their Corporate Health clinic in Hammond, IN. Provider will see ages 8 and up for Primary Care, acute episodic care, physicals, & biometric screens. Clinic offers immunizations, CLIA waived testing (rapid strep, glucose, urinalysis, cardio check, influenza), & lab work. Hours are Tuesday & Wednesday 9:00am-7:00pm and Thursday & Friday 6:00am-4:00pm, all with a 30-minute lunch break (closed on Mondays). Provider must be Board Certified and must have 2+ years of clinical experience in a Primary Care specialty to qualify for this assignment.
