Overview:
Our family medicine practice north of Columbus is seeking a Telehealth Nurse Practitioner (NP) to provide remote assessment, triage, and ongoing management for patients with a focus on chronic disease and geriatric care. This overnight role lets you directly reduce avoidable hospital visits and improve quality of life for vulnerable patients.

Position Offers:

• Schedule: Overnight shifts
  • Weekdays: 5:00 PM–8:00 AM
  • Rotating weekends: 8:00 AM–8:00 PM and 8:00 PM–8:00 AM
• Competitive salary: $110,000–125,000 per year (DOE)

Key Responsibilities:

• Perform timely clinical assessments for facility-reported acute issues, determine whether the patient can be safely managed in place, and initiate an appropriate treatment plan
• Conduct remote patient assessments via video, phone, and secure messaging, focusing on geriatric and chronically ill populations
• Perform clinical triage for incoming telehealth encounters, prioritizing care needs and determining appropriate levels of intervention
• Manage chronic conditions (e.g., diabetes, heart failure, COPD) through evidence-based treatment plans, medication management, and ongoing monitoring
• Coordinate transfer to a higher level of care when clinically warranted, and communicate with facility nursing staff and family/responsible parties
• Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate care plans and transitions
• Document each encounter same-day in the EHR with accurate coding
• Provide patient and caregiver education on disease management, medications, lifestyle modifications, and appropriate use of telehealth services

Qualifications:

• Active, current Nurse Practitioner (NP) licensure in Ohio
• Minimum 1 year of clinical experience
• Telehealth or remote patient monitoring experience, with geriatric care coordination and population health familiarity, preferred
