Job Description
Great opportunity for a Nurse Practitioner or Physician Assistant to live and work in Idaho Falls, ID for a fast growing organization delivering care to older adults in access starved communities.
- Board Certified Physician Assistant (PA) or Nurse Practitioner (NP) - FNP, ANP, ACNP or AGNP
- Must have 3+ yrs exp in Primary Care and independent patient care
- Will be focused on outcomes and not volume - prioritize quality over quantity
- Manage a patient panel, supported by the MA/LPN's, Front Office Coordinator, Clinic Manager, Medical Director, and additional teammates
- Full scope of primary care services including well acute, and chronic conditions
- Perform Annual Wellness Visits and Health Risk Assessments
- Medicare Advantage
- Must be skilled with HCC coding
