- Full-time permanent opportunity
- BC / Nurse Practitioner
- 100% Geriatric practice (PACE)
- Experience required
- Monday - Friday 7:00a - 4:30p
- Need practitioner capable of independently providing adult primary care
- Patient pop = 55+ adults
- Evaluation and treatment of acute issues for long term/SNF participants.
- EMR: TruChart
- Great location on southwest side of Philadelphia!
