Job Title: Nurse Practitioner - Mobile Healthcare for Geriatric Patients

Location: Denver, CO

Job Description:
We are seeking a dedicated and compassionate Nurse Practitioner to join our mobile healthcare team, providing exceptional primary care services to geriatric patients residing in assisted living facilities. As a Nurse Practitioner, you will have the opportunity to make a significant impact on the health and well-being of elderly patients by delivering personalized and quality care directly to their residences.

Key Responsibilities:
- Provide comprehensive mobile healthcare services to geriatric patients in various assisted living facilities in Denver, CO.
- Conduct thorough patient visits and deliver primary care services, including health assessments, diagnosis, and treatment plans.
- Order and interpret necessary diagnostic tests and collaborate with healthcare professionals to ensure accurate diagnosis and treatment.
- Manage and diagnose acute and chronic conditions, providing appropriate interventions and follow-up care.
- Educate patients and their families about health management, preventive care, and lifestyle modifications to enhance quality of life.
- Maintain detailed and accurate patient records in compliance with healthcare regulations and standards.
- Collaborate with multidisciplinary teams to develop and implement individualized care plans tailored to each patient's needs.

Qualifications:
- Active and unrestricted Nurse Practitioner Licensure in the state of Colorado.
- Experience in geriatrics and working with elderly patients is preferred.
- Familiarity with Medicare regulations and processes is preferred.
- Strong clinical skills with the ability to manage and diagnose acute conditions efficiently.
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare teams.
- Ability to work independently and make informed decisions in a mobile healthcare setting.
