NP needed for home visits in New York
Start: ASAP-ongoing
Schedule: Monday – Friday 7:45 am - 7:00 pm, Saturdays 9:15 am - 6:00 pm, Sundays 10:00 am - 6:00 pm
Will know the schedule 7-14 days in advance
Must be willing to travel through the assigned region, including into individual's homes
Minimum of two 8-hour shifts/ week
45 min per assessment
15 min call reminder prior to appointments (mandated step)
30-60 min review questions prompted on the Practitioner's Order (expected to have PO completed during this time frame)
Support the statewide Independent Practitioner Panel that issues physician orders for individuals who have undergone a Community Health Assessment and are eligible for community based long term care services and supports
Fill out a practitioner's order (PO) form, after reviewing the Community Health Assessment (CHA)
These tasks and notes are completed via secure platform
Responsible for reviewing and making recommendations on consumer assessments for long term care services and supports
Work collaboratively to monitor and address clinical quality and gaps in care
Provide clinical and educational assistance to support project initiatives
Travel time to and from patient's home must be included in hours worked and paid at hourly rate
Provider will be paid for 45 minutes if patient does not show
Training: 12 hours of paid training provided
Qualifications:
State License in New York
DEA in New York
NY State Board Certified NP
2 + years of practicing NP experience
