Overview:
Our hospice agency near North Chicago is seeking a Nurse Practitioner (NP) to provide advanced clinical care to hospice patients, focusing on pain and symptom management, palliative care planning, and support through end-of-life transitions. This full-time role offers a flexible schedule and comprehensive benefits.

Position Offers:

• Schedule: Monday–Friday, 8:00 AM–5:00 PM
  • Flexible as long as 25 visits are completed weekly
• Competitive salary: $97,000–120,000 per year (DOE)

Key Responsibilities:

• Conduct comprehensive clinical assessments of hospice patients in home and community-based settings, including physical, psychosocial, and spiritual evaluations
• Develop, implement, and adjust individualized palliative and end-of-life care plans in collaboration with the interdisciplinary team
• Manage pain and complex symptoms through medication prescribing, titration, and non-pharmacologic interventions
• Round on the inpatient unit (IPU) based on census
• Provide ongoing patient and family counseling, education, and support regarding disease progression, prognosis, and care options
• Coordinate care with physicians, nurses, social workers, chaplains, and other team members, participating in case conferences and quality improvement activities
• Document all assessments, interventions, and care plans in the EHR, ensuring compliance with CMS, Joint Commission, and Illinois state hospice regulations
• Participate in a monthly weekend on-call rotation, including inpatient rounding

Qualifications:

• Current Nurse Practitioner licensure in Illinois, APRN preferred
• Active Controlled Substance (CS) and DEA registration required
• Must be credentialed with Medicare, Medicaid, and major private insurance plans
• Prior experience in palliative care strongly preferred
• Experience providing care to patients with serious or complex illnesses preferred
• Willingness to travel regularly throughout the service area
