inFORME healthcare is looking for a dynamic, clinically adept Nurse Practitioner or Physician Assistant (new grads with hospital or ICU experience welcome ) to work with our Physician Executives to manage inpatients at a very busy inpatient sub-acute Rehab Facility that is owned and operated by a high quality healthcare company in Mishawaka, Indiana. The patient diagnostic complexity involves ortho and multiple co-morbidities. Therefore, you must be comfortable managing acute changes in place when possible.
- This position would involve a 5 day per week schedule (M-F) to manage patients, collaborate and work alongside our Physicians who round multiple times per week. May include occasional weekends and on call rotation is required.
- Additionally, you would collaborate with corporate leadership to assist in maintaining optimal momentum of the program and implementing our proprietary protocols to achieve clinical outcomes.
- This position provides dedicated support to assist with scheduling, coordination of care, and documentation to keep the clinician in front of the patient. We are seeking a dynamic and talented NP/PA to join our team.

Experience - Minimum three to five years of adult acute care/ inpatient medicine experience as a physician assistant or a nurse practitioner is required. Will also consider new grads.
Education - Educational background consistent with requirements for licensure and certification with the State of Indiana.
