Job Description
Physical/Behavioral Health NP Needed in Colorado
Target Start Date: February 2, 2026
Contract Length: Approximately 3 months (covering maternity leave)
Schedule: Mondays and Fridays, 8:00 AM – 5:00 PM (Day shifts only)
Call Details: No call
Practice Details:
Outpatient clinic located inside a homeless shelter
Patient population: Adults with co-occurring physical and behavioral health needs
Average patients per day: ~12
Common clinical focus: Foot care, wound care, infectious disease
Skills required: Physical medicine, wound management, behavioral health and substance use management (including active intoxication)
Support staff: 1 MA, 1 RN, plus therapist and case manager for referrals and therapy
Requirements:
Board Certification: FNP preferred; PMHNP considered only if very comfortable with physical health within scope
Active Colorado License (not willing to license)
Credentialing Timeframe: Fast outpatient credentialing
DEA: Required
Experience: Ability to manage both physical and behavioral health needs
Location: Fort Collins, Colorado – local candidates preferred (drive-in)
