• Start Date: ASAP
• Shift Schedule: Monday – Friday (8:00am – 5:00pm); 40-Hours per Week
• Practice Setting: Home Health/TeleHealth
• Combo of telehealth and home visits: 6pt/day for home visits (Apts approx. 45 min); 8pts per day for telehealth
• All Equipment Provided
• Conducts physical examinations, diagnosing and treating illnesses, managing chronic conditions, engaging in advanced care planning discussions, and coordinating care with other healthcare providers. Focuses on health promotion, disease prevention, and patient education to ensure continuous and holistic care for home-bound patients
• Patient Population: Geriatric
• EMR: Athena (Experience preferred or must do modules before arrival)
• Must have own transportation and a Valid State-Issued Driver's License
• Must be Board Certified in FNP or Adult NP
• BLS Required
• DEA Required
• 2-3 years experience in an acute care, home health, long-term care, or outpatient/office setting required. Palliative care experience preferred.
• Active NC License Required
