Employment Type: Permanent, Full-Time
Compensation & Schedule:
-$200,000 - $225,000 per year
- Flexibility within Monday to Friday

Key Responsibilities:
- Conduct routine monthly visits for patients in SNFs, ensuring continuity of care and timely updates to care plans.
- Collaborate with SNF staff to assess patient conditions, address needs, and adjust treatment plans as necessary.
- Perform minimal hospital rounding to provide care for patients requiring acute attention.
- Communicate effectively with patients, families, and healthcare teams to ensure optimal care and patient outcomes.
- Focus on geriatric care, primarily serving patients aged 40 and up.
Qualifications:
- Current and valid Nurse Practitioner license in the state of Illinois
- Strong communication and interpersonal skills
- Ability to work collaboratively in a healthcare team
