Job Title: Nurse Practitioner (NP)
Location: Scotia, CA
Employment Type: Permanent, Full-Time

Introduction:
We are seeking a dedicated and skilled Nurse Practitioner to join our growing team. This position provides the opportunity to deliver high-quality, patient-centered care within a collaborative and supportive environment that fosters professional development. Position Offers:
- 4/10 hour shifts, Monday through Thursday – enjoy your weekends off!
- Competitive Compensation: $139,000 - $166,000 per year (DOE)

Key Responsibilities:
- Conduct comprehensive assessments and develop individualized care plans, focusing on primary and geriatric populations
- Partner with physicians, nurses, and allied health professionals to promote positive patient outcomes
- Complete routine evaluations, diagnose conditions, and initiate appropriate treatment plans
- Track patient progress and update care strategies as needed
- Provide education to patients and families regarding preventive care and disease management
- Maintain accurate, timely clinical records
- Contribute to quality improvement efforts and assist in enhancing clinical protocols

Qualifications:
- Master’s degree in Nursing from an accredited program
- Active Nurse Practitioner license in the state of California (or in process)
- Excellent communication and relationship-building skills
- Ability to collaborate effectively within a multidisciplinary team
- Strong commitment to compassionate, evidence-based practice
