Job Type: Permanent, Full-Time
Schedule: Monday–Thursday full days, Half Day Fridays
Competitive Compensation: $140K–$165K (DOE), plus performance bonuses

Job Overview:
We are seeking a dedicated Nurse Practitioner specializing in Functional Medicine to join our Phoenix-based team. This role focuses on holistic, patient-centered care emphasizing prevention, lifestyle interventions, and individualized treatment plans. You will work with a collaborative, multidisciplinary team, engaging patients both in-person and via telehealth to improve health outcomes.

Responsibilities:
- Provide comprehensive patient care, including assessment, diagnosis, and treatment planning in line with Functional Medicine principles
- Conduct detailed patient consultations covering medical history, lifestyle, and lab result analysis
- Educate patients on health maintenance, disease prevention, nutrition, and lifestyle modifications
- Collaborate with interdisciplinary team members to create and update individualized care plans
- Maintain accurate patient records in compliance with healthcare regulations
- Stay current with research and developments in Functional Medicine
- Provide care both in-person and via telehealth platforms

Qualifications:
- Valid Arizona Nurse Practitioner license
- Active DEA license
- Functional Medicine experience is a must
- Strong clinical skills with a holistic, integrative approach
- Excellent communication, interpersonal, and patient education skills
- Ability to work independently and as part of a team
- Proficiency with electronic health records (EHR) systems
