Key Responsibilities:

Conduct patient assessments and develop individualized care plans
Manage medications and adjust treatments as needed
Educate patients on prevention, wellness, and chronic disease management
Perform well visits, sick visits, and chronic disease management
Conduct diagnostic testing and interpret results
Maintain accurate electronic health records (EHR) and use AI scribing tool for documentation
Collaborate with physicians, nurses, and other team members
Adapt to flexible scheduling and manage a 4-day work week efficiently

Qualifications:

Master’s degree in Nursing or related field
Nurse Practitioner certification with active Arizona licensure
Experience or training in chronic disease management
Strong skills in patient assessment, care planning, medication management, and patient education
Competent with electronic health records (EHR) and collaborative team practice
