This role focuses on delivering personalized, patient-centered care with an emphasis on root-cause treatment and wellness optimization. Key Responsibilities:

Provide comprehensive patient evaluations and develop personalized treatment plans
Deliver functional medicine care focused on root-cause analysis and long-term wellness
Prescribe and manage treatments in accordance with clinical guidelines
Educate patients on lifestyle, nutrition, and preventative care strategies
Monitor patient progress and adjust care plans as needed
Maintain accurate and timely documentation in the medical record
Collaborate with the care team to ensure high-quality, coordinated care

Qualifications:

Active Nurse Practitioner (NP) license in Arizona
DEA license required
Proven experience in functional medicine required
Strong clinical assessment and patient management skills
Excellent communication and patient education abilities
